They were fighting villains and Deku got punched hard on his face. Shouto is helping him out, by cooling his swollen face. Isn't that sweet?
And yes, this is totally a TodoDeku moment. Shouto will always look after his sweet boyfriend. (In my imagination, at least.)

A side note: in Japanese culture, family members and closest friends call each other by their names. As for those who are in relationships, they ask for permission to call each other's by their given name. And this, Shouto and Izuku had agreed to call by their given names.
