This form is accessed through the use of Dust Driver and a card-like trinket known as Rider Totem, which contained the very essence of a Heisei Kamen Rider throughout the history. Her own Blaze Totem is made from the Rider Totems of Kuuga, Agito, Hibiki, and Fourze, increasing her strength and her affinity towards flame-based attacks.

Her shotgun gauntlets, the Ember Celica, had its magazine filled with a special Rider Dust material that responds to her belt's command by pressing the button on her belt's bottom right compartment, marked with her own yellow color.

Blaze's Rider Form can also be said as a materialization of her own Aura, which took a form of a sturdy armor that covers her entire body. The red markings on her armor, as well as her collar and muffler, are the flame-type Rider Dust that suits her combat style and can be activated once her rage Semblance kick in, with her armor regulating the flames to prevent it from harming the user. The lilac visors can also turn red when the Semblance is activated, especially during finishers by activating her belt's command.
