You did an awesome job on this infographic, especially with how complicated the movie can be for some people, this spells the levels of the dream out very well.

but Fischer died in the fortress, which was how he was thrown into Cobb's dream world, which pretty much WAS limbo.
As referenced when they're arguing about not shooting Saito in the warehouse, someone says something along the lines of "there's nothing down there" and Arthur explains there's only what's left from whoever's been down there before (Cobb, and his and Mal's city they built)
tl;dr, I just think Fischer needs a "death" spot on Level 3
