NOTE: Feeling much better now, thanks people. Still a bit down, but it's not like that isn't uncommon in the world, huh?xDDD And not as bad as it was.How I've been feeling lately....
Answer to your questions:
I feel like crying all the time, sometimes I do.
I'm constantly tired.
I feel as though no one cares for me, but I know they do.
I feel like my art isn't good enough.
I feel like my art isn't going anywhere and won't take me places.
Everything I make doesn't make an impact.
Getting a job is the hardest it's ever been.
I'm always scared.
I'm afraid I'll fade and nothing of my work will be remembered. And neither will I.

Everyone has times when they feel down, but I'm happy your better. ^ ^
I don't know enough about your person to know if others care, I realy assume they do, though. But I know you're a great artist, and reflecting your feelings and emotions to your art makes you even better.
About the actual picture:
I like the contrast between the happy world and the outside world. Especially great are the details like the wound on the wrist (which is unobtrusive but dramatic), the fuzziness of the happy world which makes it seem like a dream or a heaven you want to enter (I hope you don't) and the bird in the sky. Even the fact that you didn't show the faces creates a great atmosphere.
I hope my englisch isn't too crappy, but the picture is just to awesome to not comment. ^ ^

Ok, this and your webcam literally made me cry....*wipes tears away* This drawing is so amazingly beautiful and holds such meaning, it's just...amazing. I think a lot of us have felt like this at one point or another. I know I have, many times. But what I find most amazing, is that you can turn that sadness into inspiration and come out with such an amazing piece of work and it leaves the rest of us in aw.
