Based on a tumblr post that explains the different fan positions that men and women used to speak to eachother.

Lovino is a young noble and overprotective brother to his little sister, who is very popular among the young men vying for her attention.
When she’s attending a ball held by one of her suitors - one in which Lovino himself was not invited to - he disguises himself as a young woman to keep an eye on her.
Upon locking eyes with a stranger - a friend of the host - he accidentally signals to him that he would like to marry him.
So begins Lovino’s fight to balance his identity as a young woman and chase off the suitor he’s slowly falling in love with and a young man trying to protect his younger sister and not let Antonio find out who he really is~
