As you can see there are two countries between them, duh! If you ever bothered to look at the map of Europe you'd know that.Also Germany and Italy aren't two KAWAII DESU men who act retarded. They are COUNTRIES. You can't ship countries.
Try learning some geography.

It does not seem offensive to me because many of the things that They happen (especially in the general attitude of the characters) it is true.

Well yes, Germany isn't next door neighbours with Italy but they are close. Some people just made a simple geography mistake, that's all.
Plus, narusasu321 has a point. I'm not a big fan but Hetalia is just a manga/anime.
No-one takes it as a true represention of the countries.
If that were the case, everyone would believe that every American eats burgers all day, have absurd ideas to solve world problems and have aliens called Tony living in their houses.
And English (British) People are all pirates, can't cook, swear a lot and would be able to curse people.....(If only we could. 
What I'm trying to say is that even if the characters share the names and qualities of the countries they represent, they are two completely different things.
Fans couple the characters, not the countries.
For example, Wikipedia has this to say on the series:

The Hetalia characters are personifications of the countries. They don't take into account the geographical location. Does America really wear a giant coat? Does Britain actually believe in fairies? No. They are lumps of rock in the sea, inanimate and as such are actually nothing like their depictions in the show.
Making pics like this will only cause drama and won't change anyone's opinion of which country is buggering who. Just let people have their fun and ignore it if you don't like it.
