In my head-canon, Ezra gets stranded on the ocean and rocky isand planet of Ahch-To after defeating Thrawn. But is later found and brought by Sabine Wren and Ashoka Tano to Lothal.

and that is a good theory and I hope that is what happens truth is I was thinking this is what happens they discover Ezar died but Asoka shows them the same vision of the past where Ezra was fighting Vader and Asoka tells them to pull young Ezra out of the vision to there time and they get Ezara from the past back instead maybe even Kanan to because the world of star wars is a strange place.

No the planet was never seen in Rebels.

But I have been at times imagined that the Ghost crew landing on Ahch-To having some moments with porgs and such.
