The Flood - I have never really embraced the genre that makes a foe more relentless and repulsive than the Flood. They were not fully eradicated last time so who can say for the Ark's destruction did away with them fully. I tired of them and their Gravemind by the end of Halo 3. As for what nearly happens to Cortana in the grips of the Gravemind that is equally insidious.
One can only wonder if they are gone forever... or if they were only a prelude to something much worse... Halo 4 has omnious underpinnings.
