okay, to clear up confusion from another perspective. Naruto is married to Hisana and as you can see she is pregnant with his child. Rukia is his sister-in-law.

Why is Rukia and Hisana both called Kuchiki? That name comes from Byakuya marrying Hisana and later adopting Rukia into the clan due to honoring Hisana's dying request, so by this pic, they'd never be Kuchiki

Hosanna is Rukia's older sister, she married Byakuya after she left Rukia behind. However after their marriage Hisana tried to find Rukia but died before she could. Byakuya promised to find her and adopted Rukia after she entered the Shinigami academy
