I love the perspective and the expression in this piece, but what I love most are the hands. The way how one is out stretched, and the other clasping the necklace(?) May I ask what the story is behind this piece?

this picture is a fanart of the series Hetaoni which is an RPG featuring the characters of the anime and manga of Hetalia. the characters are trapped in a mansion and this monster who looks similar to America's alien friend (Tony) is trying to kill all if them. Italy, through all the endings, tends to be the last one standing and has to suffer watching his friends die over and over.
this picture represents Italy's suffering of that fate. It was inspired by a picture from Count Cain and shows that he wants to join his friends but can't. the necklace is Germany's necklace which he gave him in the Hetalia manga.
