Well, piercings are not an aesthetic choice for Nino but a cultural thing. Social standing in Rivain is marked by piercings and tattoos and (since we don't get much info on Rivain, I headcanon a great bunch) to Nino they represent honor and are a really big deal for him. There's a reason he only got his lip piercing and never got another one...
So his partner Pato gave him an earring as a Satinalia gift, and Nino is still freaking out because nobody ever has honored him in this way and, while he doesn't think he deserves the honor, he's sure as hell going to try and meet Pato's expectations and live up to what that earring means. Basically, before the earring Nino was ready to take a bullet or twenty for Pato, but now he would not only take that bullet, but come back from the dead and kill any mofo that dared to lay a finger on him (◡‿◡✿) And even if they parted ways on a bad note, Nino would still wear it with pride and would cherish the memory.
So, basically, this has been life-changing for him.
