I absolutely adore this look, from the model's expression to the washed-out effect. I've been trying to get that effect but I can't quite figure it out. Do you have any tips?

sorry, I was wrong to say that as a filter, its on the right down corner of your PS for the "create new fill or adjustment layer" tab.
when you find it u can play with the color channel and or changing the layer mode.
