The main enemy in the movie is a woman who is working for the Nazi (I forget her name sorry) basically she wasn't to get to Ed and Al's world, what she calls Shambala in order to destroy it's people and take it over.

Yeah they're all alive. Envy is stuck in his dragon form and is used a transmutation to open the gate to the other side, Gluttony is now this huge white monster and Al and Wrath fight him and it ends with Al having to transmute both of them to open the gate aswell, though Wrath wanted to die.

Yeah they both die...along with Wrath too, but he ends up meeting Izumi (his mom) in the gate.
