It's alluding to World War II, with Heavy (Soviet Union/Stalin) punching the crap out of Medic (Nazi Party/Hitler).
Since, you know, Hitler tried to screw over Stalin, and the German army was steamrolled by the Russians.

You wouldn't say ''steam rolled'' if you'd know how things before at early stages of war. Even in the latter stages, Germans put up hell of a fight with the little they had.
