I already read the caption and I am commenting on this picture being ignorant of the game that you mentioned in the caption. I'm just commenting on the picture as I see it. I don't know anything about the game.
The Place. In this picture, there are people in the distance that are walking and that are near by. But in the picture, they're in the distance and aloof as to what is going on with the boy and that girl. The people are upside down, and everything seems to be in a whirl of sorts. It would make sense to me that the boy would be in a place that is more mental than physical. He is so deep into a mental state that he has very faint perceptions of reality and physics. Everything is blurry and not everything makes much sense. At least, that's what I get by the dominant gray color scheme.
Her & Him. Tears are mildly streaming down from the corner of his eyes. His arms are back, and his head is looking up at the girl. She has every bit of his attention, and he has no sense of resistance or defiance towards her. He's almost like putty in her hands. But he is hurt and scared also, and would like to be comforted by her. Her is very long and all around him; she's also naked but without shame. The boy is also in a domain that is very private and intimate of the girl; a domain where she is dominant and her strongest. But her face looks very serious and somber. Her hands are also on both sides of his head so as to keep him looking at her and maintaining eye contact. I could see her wanting to mentally strengthen him, or instill something in him, or just empower him in some way. There eyes are almost the same. So I could see the both of them being on the same page on a lot of things and/or they're very connected with each-other in that moment.
The Red Shirt. His shirt is the only thing that has color to it. I'm not sure what it could mean. Maybe he might still have a purpose later on, or it's because of a later tragedy.
I'm sorry I couldn't be more educated about the game so that I could have commented better. But thanks for reading this comment if you did. To me this picture initially looks bad; especially because dark grim colors were used to paint the girl. But the more one looks at the picture, the more it looks good and positive; at least, I don't see her being malicious towards the boy.

Such a creepy game. I only ever managed to get the meatball merge thing (which gave me nightmares for weeks). This is stunning though! Instantly recognised it 👌

Try playing "Spec Ops: The Line". Yes, it's a shooter, but the story worth it
