well since he doesn't know his name until Ichigo breaks in to Hueco Mundo, Aizen bastard mentioned his name in their meeting. he started calling him Kurosaki from that on.

Yeah, blame it at that sucker Aizen. I want Grimmjow calling Ichigo as "shinigami", since it indicates more intense about their relationship as sworn enemy (insert Satoshi/Ash vs Team Rocket scene here).
