Mod of said Genie Twi blog here, Spike gets it after suffering an injury from the main antagonist at the end of an arc called Return to Saddle Arabia (Most of it's text only. Albeit the first 11 or so parts have images from Navitas who at first accepted to make images but as time wore down it got a little too much on his shoulders and we mutually agreed to stop doing the images). It takes some previous blog knowledge to know what's going on (Genie Twi and Spike are alot closer then they are in the show for example for some events exclusive to the blog, and two of the local Saddle Arabian characters were first introduced in the blog's origin story so reading it without that would make you confused on how Twi and Spike knows these new characters already) but otherwise yeah.
Anyway, I already said it in messaging on Tumblr as well as the post itself. But I can't thank you enough Neko for this ^^ Thank you very much.

Well basically Zathir stabs him in the eye and runs away. The act angers Genie Twi and Zathir gets his just desserts. And luckily the rest of the Mane 6 are there working with the Sultana of Saddle Arabia who's a former nurse help him survive. It turns out not to be a fatal wound, but were it not for Fluttershy still having her batpony fang that we saw at the end of Bats! in Season 4, he would of lost sight in that eye. Because it managed to still hold venom that's poisonous to crops but when used as a medicine on wounds worked like a miracle cure, albeit now his eye is colored purple where usually it would be white (It doesn't show in the picture obviously since his eyes are closed).
But yeah, that's where the scar comes from.
