It mean the girl in image is Sakaki, a boy who was cute but he hasn't girlfriend. 

One day he was confession to Mariko(the girl he like)  but when he saw her...he was sad because Mariko love Kohei(boy in image)

He very angry and hate Kohai. He cursed Kohei had not love forever. But in morning he found he was a woman!! and he was pleated by Kohei.

And...Sakaki....may be love Kohei like every normal girl.

(Sakaki hate Kohei but when he was a girl he disposed him)
