There are hints in the backstory of the games that suggest Alice might suffer from acute depression -- it's known that she takes pills to ensure she only has "happy dreams," for example, whatever that might mean -- and of course in fanon there's her well-known unrequited, impossible love for Marisa. Perhaps this pic is playing off of all that and showing what it could drive Alice to do to herself; it just screams "suicide" to me.
