Interesting! Despite the comments, in fact I see nothing improper here. He has examined her, and is recommending, and providing, some kind of medical help. We don't even know(!!) that a nurse hasn't been present throughout. Nor does her facial expression look fearful, or disgusted, or anything notably negative. She seems almost to be fully focused on that medicine bottle.
That he could be just a DOM is our imagination, nothing more.
I shan't deny that she is extremely easy to look at, nor that her hand position makes some implications. Still....

Curious to know why her hands are so pale (white from gripping so tightly - the 'digging in' marks on her own breasts are really well done)
Interesting that she's allowed a little modesty in this shot - all the more 'sexy' for this I think.

There's no real reason why they are so pale except for the fact that I was playing with the contrast and color to keep them from vanishing into her breast. For instance, take a squint at the art and imagine the hands being the same color and contrast as her breast and you'll be able to see how easily they could fade away and then perhaps you'll see what I mean. Just perhaps.
