Just a plague doctor as a pony.

Plague doctors were mainly second-rate and novice physicians who usually couldn't own their own medical businesses. They 'appeared' during plagues and treated sick patients. They wore masks with beaks. The beak contained aromas such as amber, cloves, and storax to block out the smell of deceased people. Some carried sticks to direct people. (It was hard to talk through their masks)

Basically they were the Grim Reaper. If you saw one of them when you were sick, it was almost certain you were to die.
