Does the nervous system work both ways? If the machine is damaged, does the pilot feel physical pain as a result?

Of course. Although I would think that they'd engineer it to be perceived differently. But the pilot would know what part was damaged and in what way.

Yess, I was wondering about that too considering how sensitive the spine is and all those pesky nerves. I guess a bit of coding goes a long way!
I can't help but think that there could be some horrible event where so much damage is sustained to the vessel that it overrides/malfunctions the connection between pilot and mecha and have some horrid consequences.
