It's not very complicated, though nowadays there are far less clunky options you may want to consider, such as sketchup.

Part of the point of ico files having multiple images is to allow you to replace the smaller version with a more appropriate graphic. Most of the time an icon that looked good at 48x48 or 256x256 won't look too good when scaled down to 16x16, mainly because all the details get blurred and become difficult to see.

Axialis Icon Workshop. You have to purchase it though...but it's the only one I've seen that supports 256x256 PNG compressed.
