I don't think a prosthetic arm would have any significant effect on your identity. It's not like an amputee is a completely different person from what they were before. Being a werewolf on the other hand is a much more drastic change. You are no longer what most people would even consider human. Much of your physiology has been altered beyond recognition.
But does that make the werewolf here a different person? Well, personhood is a psychological phenomena, a product of the brain. If this werewolf retains his/her personality, memories, and psyche, then I would say he/she is the same person regardless of outward appearances. However, if this werewolf behaves in a manner that goes against these attributes, like how some werewolves in mythology go feral, and have no control over their actions, then in that case, I would say the werewolf is not the same person anymore.
These are not easy questions to answer, but it sure is fascinating to think about.
