Looks alot like Mignola's Clown Cenobite or Pavi Largo from Repo! The Genetic Opera. It's very well rendered but honestly this is entry-level extreme. When you get into Coldharbour, Realm of Chaos, Clive Barker territory everything looks like the last log of the Event Horizon. Basically bloody, eyeless and smiling with an erection replaced by a knife.
