Description I will be using Photoshop CS2 for this tutorial.

1. Go to layer > new adjustment layer > curves, select "red" from the drop-down list and fill this in:
Point 1: 45, 44
Point 2: 197, 204

Now select "green" from the list and fill this in:
Point 1: 167, 168

Finally, select "blue" from the list and fill this in:
Point 1: 43, 41
Point 2: 228, 225

2. Go to layer > new adjustment layer > colour balance, check "preserve luminosity" (if it hasn't been checked yet) and fill this in:
Shadows: +7, -14, -12
Midtones: -13, +2, +10
Highlights: +4, +1, +5

3. Go to layer > new adjustment layer > selective colour, check "relative" and fill this in:
Neutrals: +1, -2, -1, -2
Blacks: -12, -11, -21, +7

4. Go to layer > new adjustment layer > levels, select "red" from the drop-down list and fill this in:
Input: 8, 1.02, 252
Output: 5, 255

Now select "green" from the list and fill this in:
Input: 10, 1.05, 255
Output: 4, 255

Finally, select "blue" from the list and fill this in:
Input: 2, 1.00, 247
Output: 20, 221

5. Now go to layer > new adjustment layer > colour balance again, make sure that "preserve luminosity" is still checked and fill this in:
Shadows: 0, 0, -4
Midtones: +7, -4, -8
Highlights: 0, 0, 0

6. Duplicate your background/image layer and drag it to the top. Set it to screen at 30%. Now go to image > adjustments > desaturate.

7. Duplicate your background/image layer again and drag it to the top. Set it to soft light at 50% and desaturate it.

Now merge the layers and you're done!
