I am awful at giving critique's but I'll give it a try.
To start off, great job making this piece! It looks really good! The vision of Simon's past is clear to see with the cut marks and pills in the background. I like the addition of the blood directly behind Simon himself that is splattering away from his body I think it's a good way to help show the downward spiral he is on.
I do wish that you had cleaned the line art up bit and hadn't added the dripping blood on top.
