Very painfully. It is, in fact, designed to hurt the wielder because it is a bound goblin. He isn't happy about being bound so he choose to be uncomfortable to use. The protagonist in the novel get around it by wrapping a chain around the handle so his hand doesn't get stabbed too much.

I actually went and read Worm on your suggestion (hope you get around to making that Leviathan piece you talked about, since I'd like to see how our interpretations line up), and started on Pact after, and I reached this part in the novel right after I looked at this piece. Matches up great, in my opinion!

It's supposed to be, the sword is a subjugated monstrous goblin that has taken this form as part of it's binding. It's an insanely useful weapon for the protagonist of the story (Pact by Wildbow) but there's no way to wield it without hurting yourself.

a monstrous goblin? for a powerful sword? well, thats an interesting one.
usually goblins are weak but not this time it seem.

The goblins in Pact are one of my fav bunch of characters. I love the world building and magic system there.... the actual story, not so much. It feels like Constantine (the comic) or maybe even The Sandman comics. Sort of a modern horror/fantasy setting.

though the comparison with the sandman and constantine makes me back off. i just don't like horror at all. especially neil gaiman's writing, too uncomfortable.

Pact might be too intense for you then. There's quite a bit of body horror. I thought Gaiman's stuff was alright, though.

Part of the beauty of it is... it feels at times like reading a horror novel from both ends of the spectrum. As both victim and monster.
Pact is wonderful, surreal and yet very very grounded. Though admittedly it does get particularly uncomfortable at some points.

That reaction... There's a beauty in the craft, in every bit of the dark and terrifying things. Not appreciating that beauty make it easy to see why you don't enjoy the particular artform of Horror.
Not to berate you for it or anything though. Sometimes seeing the beauty in something ugly is just not something that some will, or are willing to do.
