shouldn't it work more if it was "Who you are" rather then "how you are" using that makes it sound like their asking if the person is okay

The original sentence in Italian means "how you are physically, what is your appearance"
I don't do the translation (I'm not good in English) it's made by my friend.

"Who you are" may have been more appropriate for this situation, but "how you are" could work too! Like Mr J. is saying "I know what you're like", kinda thing
