I have an idea of how Malak's punishment went down.

Revan is standing on the bridge of his ship, his hands behind his back looking out onto the stars. Behind him, on his knees is Malak, holding his limp right arm, his left eye bruised. Standing either side of him are two Sith loyal to Revan. A Devaronian, and a hooded Human, with their face covered.
Revan says: "It was bold of you to question me so openly Malak." He looks over his shoulder. "It was even bolder to challenge me."
Close up of Malak's bruised face, frowning in frustration and anger. Revan back to looking out the window. "You can't be surprised. Even as Jedi I was always your better. What made you think now would be any different?"
Side shot of the two. "While killing you would be a waste, *apprentice*. Like in war, there are always consequences..."
Close up of Revan's hand, Lightsaber gripped.  "In both victory..."
Close up of Revan's mask, now illuminated by the purple glow of the blade. "...And defeat."
Side shot. Revan mid swing, his left hand still behind his back, as his blade slices Malak across the mouth. "Arrrrrr!" Malak cries out the best he can.
Shot of Revan both hands behind his back at an upward angle. Malak's head dipped forward in pain, his mouth smoking. "Next time you gamble, Lord Malak, ensure you can suceed."
Revan walks past his Sith, Malak still on the flood. "Otherwise I may not bother being merciful again... Call the infirmary."
