I don't know the proper name for the fish that's tickling them on the bellies, but I do know that their used in indoor spas eating dead skin cells.

Well the fish in the pic is called cleaner fish
As for what you said these are not the fish you are ticking of the one used in spa are of a speice called nibble fish
Both have same nisch but in one lives in sea while the other is fresh water fish,there are also the colouration
