Um, she seems more emotionally unstable than insane, really. I mean, her sister basically told her to stay in the basement for 495 years. She complied out of love for her sister until the Extra Stage of EoSD (well, she technically watched Reimu and Marisa fight Remilia)...
My experience with helping babysit her (Sakuya is not always readily available to do so) is that she is really a sweet little girl once you get to know her. Oh, and she has slightly more control of her power than you may think. She did save Gensokyo from a meteor, after all. She is still not the best at controlling her power for the simple reason of what her power is...

I was talking more from seeing that some people portray their love for each other as a bit...more...than sisterly. They are cute together as sisters. Any closer depends on who you ask...
