Assuming you have a program already set to open FLV files, you simple do the following:
1. Save the icon to a known location
2. Go to Start > Control Panel > Folder Options
3. Click the File types tab
4. Scroll down until you find FLV
5. Click Advanced
6. Click Change Icon
7. Click Browse
8. Find where you saved this icon and select it.
9. Click OK on all of the open windows.
10. Enjoy
