They didn't find out she was a girl until one day, when the Host Club ended for the day, some of her hair pins fell out. They're identical to her hair color, so you can't even see them. Her voice is more on the feminine side, so she has do disguise her voice; not much effort has to be put in to doing that. Nobody knows how she does it with her long hair of hers. Shortly after she joined the Host Club, she started to develop a crush on Kaoru, noticing his kind, caring personality. (She probably would've fallen for Tamaki if he wasn't so damn flirty.)
