Allison was annoyed about being diagnosed with scoliosis earlier this year, and hated the back brace she was prescribed. Instead of wearing her brace for 23 hours a day, as directed, she often spent weeks without it. Her doctor wasn't happy with her progress and assumed that she was cheating, but he was close to retirement and didn't want to spend the last few months of his career fighting with a self-destructive young woman.

During Allison's final appointment with her doctor, he told her that Dr. Gina Phillips would be taking over her care. He warned Allison of Dr. Phillips' reputation, saying that she did not tolerate non-compliance in her patients. Allison feigned innocence, not wanting to admit that she had cheated so frequently with her brace.

Her first appointment with Dr. Phillips was short: the doctor reviewed Allison's x-rays, said that her spine should be much straighter than it was, gave Allison a stern warning to wear her brace as directed, and made a follow-up appointment for a month later. Over the next month, Allison spent the majority of her time out of her brace, and tried to ignore the occasional back pain she was feeling.

Allison's follow-up appointment was today. Dr. Phillips was very unhappy when she saw updated x-rays of Allison's back; the curvature of her spine was worse than in her initial appointment. Dr. Phillips told Allison that it was obvious she hadn't been wearing her brace almost at all, and that she was risking damage to her spinal cord and internal organs due to the shape of her spine.

Dr. Phillips said that she had no choice but to adjust Allison's treatment to ensure her compliance, and that she recently learned of the success one of her colleagues, Dr. Estelle Gippson, had with one of her "problem" patients.

Allison was shocked to hear that her brace would be upgraded to a cast immediately. She told Dr. Phillips that she would wear her brace as directed, that putting her in a cast wasn't necessary, but her pleas fell on deaf ears as two ortho techs arrived to escort her to the cast room.

The cast techs applied stockinette and padding to her torso, around her neck, and across her forehead, and Allison was very confused when the cast techs pulled pieces of stockinette up her arms. She frantically asked what was happening, and started to panic when she was told that her arms would be included in the cast -- Dr. Phillips had ordered this to ensure there was no possibility of Allison removing the cast herself. She begged to keep the use of her arms, but was told that the doctor's orders were non-negotiable, and she would be sedated if she didn't cooperate. Allison held back tears as the casting material was wrapped around her body, then as the cast techs crossed her arms over the fiberglass around her torso. They moved her arms away from her body, quickly wrapped them in casting material, then crossed her arms over her torso again. They wrapped more fiberglass starting at her hips, around her casted arms, over her shoulders, up to her chin, and across her forehead, completely encasing her arms and upper body.

Allison followed instructions and didn't try to move as she felt the various parts of the cast warm up and then cool off as the fiberglass set. Once the cast had hardened sufficiently, the cast techs released her from the traction apparatus. She was surprised to feel that the tension in her back didn't change at all, then realized that she was now held in the cast instead. She avoided trying to move her arms, trying to deny the reality of what was happening to her, as the ortho techs helped her to her feet and escorted her back to Dr. Phillips' office.

Upon seeing Dr. Phillips again, Allison pleaded for her arms to be released from the cast, promising to comply with this treatment. Dr. Phillips explained, in a curt dismissive tone, that her cast would not be altered in any way except for periodic cast changes for hygiene reasons. She concluded the appointment by telling Allison to schedule a follow-up in two weeks, then monthly checkups after that, and that her cast might be reduced if she showed significant progress after six months.
