Well, I usually start with a very loose sketch, just to get the feel of what I want make sure that i get everything where I want, and on I make with the details. And then when i'm doing that I always get ideas on little stuff, sights gags etc,that I add in so when i'm done I usually get everything that I wanted.

interesting, seems like an organic process. I always feel mine is a bit forced.

The only reason im sticking to black and white at the moment is because my wacom pen nib is wearing dangerously short. So i'm trying to preserve it as long as possible which prob means no colour till I get a new nib.
