The horns are attached to his headband like Jiriya's sage headband O.o That's why they match the same coloration. The horned forehead protector is rather popular among sages it seems.

we have never seen more than a shadow of the sage, so we don't know if he even had a headband (considering that system is meant to have been from the days after the villages foundings, Madara and various pre-village ninjas are shown without them) Also, Jiraiya's headband has never been said to do with his headband.

Considering Ginkaku and Kinkaku are also meant to be descendants of the sage of six paths, and they have unexplained horns, I'm more willing to believe the sage's were just something he had on his body for one reason or another.
