I filled a large garlic powder lid up with tin foil, took the metal hoop and put some plastic layers on the back, glued it to the foil side of the lid, and sculpted everything around it. Once it was dry I attached the chain to the back with thumbtacks.
That was pretty much it. It's been a good 4 years since I made this though so I can't really remember the specifics.

I got a garlic powder lid, covered it in foil and hot-glue, covered it in Paper clay, put the screen on, and kind of just sculpted it until I got it the way I wanted it to look.

Paper Clay is a very smooth, ready-made, easily workable material. While paper Mach'e takes quite awhile to prepare, doesn't go on very well, and is very hard to do much with but sand once it's done drying. Plus it will have little chunks in it no matter what you do.
Paper mach'e is good for adding strength to projects, but if you want a nice smooth surface it's best to use paperclay.
