I don't remember when, but its right around the time Bulma goes to meet Trunks and Gohan to find the destroyed time machine with the cell egg inside, right before she leaves capsule corp in fact.

Her eyes are open in one episode (and manga too), it's the time Bulma asks her mom to take care of kid trunks while she visits trunks "all grown up".

I can't remember if she ever opens her eyes or not.

Yep, she did; once. When Bulma said she was going to meet her son 'all grown up' just after she told her mom to look after trunks. I guess the confusion caused her to briefly open her eyes.
