so technically, Perma was taken through a 'cave' that intersects the kingdom caves, Perma's neck was slashed, so naturally she only has a couple of minutes to live. But as a child, she would want the pain to end (not to mention her bipolar personality lead her to attempted suicide)
Smoke saw Perma in pain, so she ended the pain along with her life, not knowing about Verglas' existence.
