Yep. The gameplay takes place when *spoiler alert* the kid is in the hospital after being bit by Fredbear. The nightmare animatronics are the way he sees them due to being taunted by his brother.

I know what you mean but remember that he's pretty young and little kids tend to have huge imaginations. So it's probably not the fact he's mental but more to be he just has a huge imagination.
