its still was said based on that topic you posted by $realitysquared that its a case to case basis. and her work wasn't just about that. there's a more deeper meaning that she is aiming for people to see. she is not encouraging self-mutilation but projecting an idea about what a person does. what i can see in this picture and with her description is about people taking up vanity too much, too concerned about physical beauty, not really thinking of consequences of what they do (for example plastic surgery).
what i see here is that you just went, looked at the picture and cried --"self mutilation!!!" without really looking at what it is.
