Technically, it would be part 3. This comic is essentially in two 'chapters'. As many others have said though, a third piggy would be a cute spin on the '3 little pigs' motif. My favourite suggestion would be the gym's owner idea (but female) and have her start out as muscled. Not necessarily a bodybuilder, but that might prove rather satisfying.
And then part 4 could feature 'the Big Bad Wolf'; a female lawyer (a short older woman in her 40s or 50s) who wants to blow down the professor's mansion on charges of inhumane cruelty and well, you get the idea...
I can only hope that Willix does return to this comic again one day (and his latest post proves the original WB cartoon remains an inspiration), it is an absolute beauty.

it is possible to make option of the third girl who of very poor family and starves. she goes and asks food, but all refuse to it around... once, it appears at the lock where the kind doctor actively fattens two girls... the doctor listened to its story, thus her story heard also two fatties. the history makes the strongest impression on Satsuki, and both girls help the doctor to fatten it

Did you ever think to introduce a third girl to this adventure? I think it would be awesome. I'm a huge fan of your work. I even made a tribute of drawing that I attempt to upload some day. Keep the nice work!

I think it's just the angle. Satsuki is definetly bigger. She's now ALL boobs and butt -- she was nowheare near that state when the comic was cut chort.
As for Mei, We are seeing her straight on frontal; that is something we hadn't seen since she was still able to walk. Remember, she is primarily belly-heavy. We are looking straight into her belly; so it is projecting directly at us. This is mearly a forced perspective illusion.
