Sailor Moon, age 14: Beginning her training as Sailor Moon, Usagi didn't know what to do and was often scared. This lead to having her being rescued by the mysterious Tuxedo Mask(Mamoru Chiba) and even her friends. She became comfortable in her role as Sailor Moon and her brooches evolve as she continues gaining in strength. Her Moon Crystal Star Brooch appeared in Act 13 of the manga after the fight with Metalia and in episode 51 of the Classic anime. She gains the Cutie Moon Rod to perform the Moon Princess Halation attack. Usagi would later gain the Moon Cosmic Heart Brooch in Act 23 of the manga from Neo Queen Serenity and episode 91 when the love between Usagi and Mamoru leads to it's new brooch's formation as well as the Spiral Heart Rod that enables her to use the Moon Spiral Heart Attack.
Princess Sailor Moon, age 14: Exclusive to the live action version. Serenity has full control of this form, causing an internal conflict between her and Usagi. Her form has a navy blue fuku with a pearl choker, a pink bow, a silver locket with a pink crystal, a pearl crown, red odango covers surrounded by pearls, pearl belts, lacy sleeves, a lacy back bow and lacy petticoats. She wears white boots.
Holy Grail Super Sailor Moon, age 15: Using the Holy Grail, Sailor Moon evolves into her Super Sailor Moon form to utilize her more stronger attack, Rainbow Moon Heart Ache. How she gains the Holy Grail is different in adaptations. In the manga, the Inner Senshi and the three Outers contributed their powers to form it and this is given to Sailor Moon. In Crystal, Sailor Moon, Tuxedo Mask and Sailor Chibi Moon held hands to form the Holy Grail and it's powers were used to free the possessed Inner and Outer senshis from Cyprine and Ptitol's influence. The Inners, Outer three and Chibi Moon contributed their powers for Usagi to become Super Sailor Moon. In the Classic anime episode 111 of the Infinity Arc, the talismans belonging to the Outer three(Space Sword for Uranus, Deep Mirror for Neptune and Garnet Rod for Pluto) were used to create the Holy Grail. Usagi had to compete with Eudial in order to get the grail before her. Unlike in both the manga and Crystal, Usagi suffers from power drain after each use. She eventually loses the Grail in Episode 125 when she was tricked into giving it to Mistress 9 against the wishes of both Sailors Uranus and Neptune who both suspected a trap. Usagi would become Super Sailor Moon one last time when she was loaned the powers from the four Inners and the two Outers to assist Sailor Saturn in defeating Master Pharaoh 90
Super Sailor Moon(Pegasus version), age 16: Usagi, along with Chibiusa gains this form when Pegasus grants them their new brooches in Arc 34 of the manga and Episode 130 of the Classic anime. Usagi gains the Moon Kaleidoscopic to perform the Moon Gorgeous Meditation attack. Her fuku is still elegantly ornate with the exception of her back bow which is shorter than from the Holy Grail.
Eternal Sailor Moon, age 17: Almost nothing remains the same in Usagi's form as Eternal Sailor Moon. She gains this form in Act 42 of the manga with the resolve to defeat Nehellenia and in Episode 168 when all of the senshis combined their powers for her to become her Eternal form. Diana mentioned Eternal Sailor Moon is the closest to Neo Queen Serenity in power. In the manga, Eternal Sailor Moon gains a pair of functional angel wings, while the classic anime had her with wing, but only used as decoration. In addition to the Holy Grail evolving to the Holy Moon Calice, she uses it along with the Silver Moon Crystal to perform her attacks.
Sailor Cosmos, age 26: Mysterious and beautiful. In the Stars manga during the last battle and disguised as Chibi Chibi(first anime had her revealed to be Galaxia's Saffer Crystal), Cosmos guides her past self Eternal Sailor Moon to the Galaxy Cauldron. She tells her to destroy it at once, but Eternal Sailor Moon refused knowing that if she did, no more stars will be reborn. She chooses to sacrifice herself to the Cauldron by jumping in it, making Sailor Cosmos realize that was the right decision. After revealing to the Asteroid Senshi about her sad history and the fight against Sailor Chaos, she whisks them away back to the future.
