There is a story behind it. And Kaoru is genderless. In the original story, Kaoru was stolen by one of the nurses when she and Koichi were born. So she wouldn't be caught, since Kaoru had the same red eyes as Koi, her mother made her wear blue contacts and dress as a boy.
