well I made a quick effort
you can download the file here
if you get a message on opening the file, it's becoz you need the modified reference image as well (I changed the colors for easier use)

File contents:
Open it in max and you will find 3 objects of wich 2 visible:
a plane used as a floor
an editable poly in the shape of your logo
a line object in the shape of the logo
when you take a good look at the editable poly you'll see that it might need a bit more tweaking.
It's best that you hide that object and just work with a copy of the line object (shift+move it to copy it)
The line object has it's vertex ticks on. you cna edit the shape of your logo line by moving those vertices. When satisfied just rightclick and convert to editable poly. Then go to sublevel 3 (borders) and select it. click cap in the sublevel menu and then extrude in sublevel 4 (polygon level)
materials are up to you

There you go, that's my weak point. I totally suck at texturing.
The way I do it is use the edit mesh modifier and then assign material ID's to different parts of your mesh.
You can find a good tutorial on this topic at [link]
I hope this helped you a bit. If you want to use unwrap UVW modifier I'd have to disappoint you as I'm still trying to figure out how this works
