Yeah, painted 100% digital in photoshop in about 4-5 hours (at a guess).
A combination of practice, custom brushes (from ImaginFX mag, great for blending tone), and shade loosely based on ref from a mag 
Im quite pleased with it as [this] is the kind of thing I was doing in sept last year.

Yeah I toyed with that for a while but didn't really like the effect, plus its very time consuming. I also tried Adobe illustrator's blob tool (smooths the lines as you draw) which is good for comic style art.
Now I just draw them free hand using a very dark tone that matches the skin tone I intend to use (or rather I try too) the lines appear smoother in the final image than they actually are because the final image is actually only 25% of the original scale.
Basically open a really big document (mine are usually around the 3000-4000px area but depends on image) and draw free hand while zoomed at 25-50%, zooming to 100% for details. If needed I also use the smudge tool if the linework still looks rough at 25%... but only use this very sparingly.
(again I use a custom brush but I'm not sure where it was from. The best thing to do is test out as many brushes as you can till you get one you like)
