Desde un punto de vista médico, la paidofilia o pedofilia es una parafilia que consiste en que la excitación o el placer sexual se obtienen, principalmente, a través de actividades o fantasías sexuales con niños de, generalmente, entre 8 y 12 años.[1] A la persona que padece pedofilia se le denomina pedófilo, un individuo de, al menos, 16 años que se entretiene sexualmente con menores de 13 y respecto de los que mantiene una diferencia de edad de, por lo menos, cinco años.[2]
La pedofilia es un rasgo multifactorial en la personalidad del que la padece, y se compone de aspectos mentales, institucionales, de actividad, de educación sexual, de violencia, de control de las pulsiones, etc. En este sentido, se suelen distinguir dos tipos de pedofilia, una primaria o esencial, muy arraigada en el sujeto, y otra secundaria (u otras), que aparecería motivada por factores circunstanciales.
Por lo demás, en determinados casos en que la relación entre el pedófilo y el menor se prolonga en el tiempo, puede haber por parte del adulto un enamoramiento real con esa persona a la que él considera como su joven pareja, sobre todo cuando esta se halla en la edad de paso entre la infancia y la pubertad.[3]
Existen, a este respecto, diversas asociaciones de pedófilos que reivindican la pedofilia como una forma más de vivir la sexualidad humana y que, en consecuencia, debe ser aceptada con naturalidad por parte de la sociedad.[4]
Las conductas pedófilas son muy heterogéneas, desde casos inofensivos o casi inofensivos, hasta aquellos en que alcanzan niveles que entran dentro de lo criminal. A la actividad sexual de un pedófilo con un menor de 13 años se lo conoce con el nombre de abuso sexual infantil o pederastia[5] (palabra que, etimológicamente, significa lo mismo que pedofilia).[6

Siguiendo con el tono de Erotica, Madonna publicó un libro íntegramente sobre sexo. El diseño estuvo a cargo de la propia Madonna y de Baron & Baron Inc. (cuyos integrantes eran Fabien Baron y el fotógrafo Siung Fat Tjia), que también diseñaron el artwork para el álbum y el sencillo homónimo. Se puede decir que usaron el estilo Warholiano.

En un principio se iba a llamar X, pero al enterarse que se estrenaría la película de Spike Lee Malcolm X, sus productores cambiaron el título por el actual. Sin embargo, el diseño original de la tapa con la X se mantuvo en la contratapa.
Surgió también la idea de hacer el libro en forma ovalada, pero la impresión y la manufactura iban a suponer un aumento en los costos, por lo que esto fue descartado.
