How do you get those really clean outlines, I can't seem to get them when I do CS3, I'll get a kind of uneven black line that I'll usually have to do over.

I tried using the fill tool, but as it turns out, the fill tool doesn't actually fill everything, it actually leaves a lot of white behind.

I use paint tool Sai for all my art. I mostly keep smooth lines because I make the image itself very large -- and also knock up the resolution.
More pixels = smoother lines.
