My first shot at drawing realistic(ish) people traditionally since like...self-portraits in art appreciation class two years ago. It's just a random sketch that I ended up finishing.Adjusted the levels in Photoshop.
Tools : HB & 4H pencil.

On more thing I see is his nose.
You are an excellent shader. And from what I can see a great burnisher. Your perspective is excellent.
However, his nose does not appear to be in 3D space. XD
You have the light coming from the top left, but his nose is dark on all sides. Put a little shading in the nose.

The reason the eyes look wrong is because anime is an art style unto itself. It is a characterization of life quite similar to impressionism.
Anime character's eyes are exaggerated to express deep emotion (or in Ulquiorra's case extreme lack of emotion.) You trying to do a realistic drawing of an anime is similar to trying to redo Picasso in realism.
If you want to make the picture look right, you need to make his eyes bigger. you could always try doing a combination of realism and anime (called romanticism. a combination of realism and fantasy.)

Epic shading.. is epic. I think the hair and neck got a little blurry but meh that's probably my screen. Much lurve to the skull/helm/bone thnigy. C:

haiiirr. haiiiiirrrr. *___* looks amazing. it -might- help the eyes if you shaded darker under the brows... they don't look too bad in the first place. :<

the shading is amazing, and his hair looks fantastic. other than the eye problem, this is excellent

His eyes are hard to get right.
But as far as realism goes, you've done an amazing job!
The shading is so masterfully done!
He was certainly a good choice since it's always difficult to do shading or even coloring on abnormally pale skin. 
The hair looks absolutely perfect!
